NEW PATIENT PROCEDURE
The initial visit at our clinic will take approximately 60 minutes. Please plan to arrive 15 minutes early to fill out paperwork or click the link below to complete the intake information:
WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?
We accept most insurances including: Blue Cross, Aetna, Cigna, Workers Compensation and Auto Insurance. Give us a call and we will check your benefits and ensure we are in network with your insurance company.
DO I NEED A DOCTOR’S REFERRAL?
We do not require a doctor's referral but your insurance might. If you call in with your insurance information we would be happy to verify your benefits.
